Enrollment fee: Free through December 31, 2026. Starting January 1st, there will be a one-time $30 enrollment fee per household for new enrollments.
Additional Children: $10/month for each child ages 0-23 after the fourth.
Please note that while you can cancel at any time, patient panels are intentionally limited and re-enrollment may not be possible.

What’s not included:
We focus on primary care. We recommend pairing your membership with a high-deductible health plan to cover what the membership doesn’t:
- Specialist care
- Hospital and emergency care
- Imaging (X-ray, MRI, CT, ultrasound)
- Obstetric care
- Vaccinations (county public health offices and most pharmacies offer routine vaccines)
- DOT physicals
Osteopathic Manipulation
With the DPC model, Dr. Paul Weaver will see patients at West Salem Family Practice on a $150 per-visit basis (not via a monthly membership). Patients of Dr. Weaver pay only for the visits and treatments they receive. Patients of Dr. Nichole Scherlie and Dr. John Guy may also receive OMT on a $150 per-visit basis.
A note about availability
Direct Primary Care only works when each physician’s patient panel is intentionally limited — that’s what makes longer visits, same-day appointments, and direct access possible.
Current patients have priority access to enroll in our DPC program beginning August 1st, 2026 and will begin receiving this level of care on October 1st, 2026. At that time, if space remains available, enrollment will be open to the general public.
